When to Refer a Patient

Thank you for considering Big Stretch Canine Physical Therapy for your patients.

 

This guide provides the information needed to coordinate care and create an appropriate rehabilitation plan. Along with the referral form, please send any relevant veterinary records, including medical history, imaging, surgical notes, or other records related to your dog's current condition.

 

Having this information ahead of their evaluation helps me better understand where they've been, where they are now, and how we can best support their next steps. Please send their records to hello@bigstretchcaninept.com.
